Details: By signing on the other side, I accept the offer (which is pre-approved, assuming by continued excellent credit) to transform and "upgrade" the Chase Credit Card I now carry to Chase Prestige Credit Card status immediately to ensure it is in place when my Chase Credit Card expires. Chase Credit Card will come with all the upgrade services outlined in the mailer. My Chase Prestige Credit Card Credentials including special stickers to be affixed to my Credit Card, which signifies my Chase Prestige Card status, will be sent to me promptly after I return this form. The annual membership fee of just $49 for Chase Prestige Card services will be automatically charged to my Chase Credit Card account as long as it is in good standing and for as long as I wish to remain a member without me providing my account number to anyone. This fee is in additional to any annual membership fee which Chase may charge for the issuance of the card itself. But if I decide at any time, for any reason, that Chase Prestige Credit Card status isn't for me, I may simply cancel and received a refund of my Chase Prestige Card membership fee... and my existing Chase Credit Card account will remain open. No questions asked or strings attached.
