There's also a large OFFICIAL NOTICE notation, a notice DO NOT FOLD, and a box below it that includes the following information:

This ID# will expire on	10/5/2006 at 11:59 PM

The most obvious feature visible upon opening this extremely important notice is in large print on the right-hand side:

My eyes jump down to the pre-printed disclaimer at the bottom:
